Diamond Village Care Home is a premier adult residential facility servicing adults ages 18-59 years with Developmental Disabilities. We welcome statewide Self-Determination Program participants and all other individuals with similar principles and values. We pride ourselves in providing outcome-based care and promoting quality living to adults with developmental disabilities who desire to be fully integrated into their community and require assisted living services. Our goal is to be flexible enough to accomodate individual needs while remaining within the scope of our license and other regulations in place. At Diamond Village Care Home, each participants' Individual Service Plan is very important to our day-to-day operations. ------------------------------------------------------------------------------ Our Mission: To be the premier Care Home in the Elk Grove/Sacramento area that provides quality care and access to all-inclusive services. Our Vision: Diamond Village Care Home will provide all-inclusive services with the purpose of consolidating the community resource needs for clients with Developmental Disabilities. Our Services Menu includes: access to 24/7 Licensed Healthcare Professionals, Chronic Health Condition Education/Assistance Our Programs include: Leisure travel with at least 3 major trips annually for enrichment purposes, Assisted Living Waiver program, Restricted Health Conditions (we may apply for an individual exception from licensing on a case by case basis)
