Rhiannon has a passion for guiding individuals, and their families, through the Person Centered Planning process and Self Determination. She supports clients through each step in the transition to SDP with the goal of making the process as easy as possible.
She is here to help find the perfect supports for you, or your loved ones, own unique needs. Her goal is to help improve your life and in doing so make a “life leap”.
Rhiannon’s education includes an undergraduate degree in English, which gives her excellent writing skills. She holds a Masters Degree in Information Sciences, which honed her research and administrative strengths. Her professional experience includes over a decade of consulting individuals and providing them support to reach their goals.
