Rachel Credo, MS

Rachel Credo is the founder of "A Pathway to Autonomy, LLC", abbreviated as "APTA." Rachel possesses extensive expertise in serving individuals with developmental disabilities, accumulated over a span of 16 years. This experience includes roles such as direct support professional, facility manager, facility administrator, and QIDP. Additionally, Rachel has dedicated the past 6 years to providing exceptional service coordination (SC) to Westside Regional Center community. Rachel possesses certifications as an IF and PCP planner. As an SC, Rachel assisted around 25 to 30 clients, comprising of both children and adults in transitioning to the Self-Determination Program (SDP) service delivery. Rachel provided a thorough and meticulous explanation and support of the SDP to all clients served and their families, thus ensuring a smooth transition. Clients and families that Rachel helped described her as compassionate, professional, considerate, respectful, and knowledgeable.
