Heather Crocker

Hello! My name is Heather Crocker and I am an Independent Facilitator at alift llc. I am passionate about Person Centered Planning and have supported individuals through every part of the Self Determination Process. I reside in Windsor and currently serve clients in the North Bay Regional Center catchment areas of Sonoma, Napa, and Solano counties. I have a background in leadership and people development and am passionate about helping individuals navigate towards their goals to live a better life. I am the parent of 2 children, one of whom was served by the Regional Center at a young age and that sparked my passion around this work. At alift llc, supporting people is what we do. We provide unbiased, comprehensive person-centered services. By working collaboratively with individuals and their circle of support, we facilitate a process that informs the person’s needs and reflects their hopes and dreams.
